Registration Information and Fees

Registration:
TO ENROLL IN THE JEFFERSON COUNTY SCHOOL SYSTEM, YOU MUST HAVE THE FOLLOWING:
1. ORIGINAL STATE OF ALABAMA IMMUNIZATION FORM
(Blue and Pink) from Health Department, previous school or family physician

2. VERIFICATION OF RESIDENCE - Three forms of verification are required.
1. Lease or deed to residence (or mortgage statement or payment book) with the name and address of the enrolling adult or spouse.
2. Two current primary (power, gas, water, phone (land line) utility statements in the name of enrolling adult or his/her spouse, identifying
the residence as the service address.
NO cell phone bills, deposit slips, checks, or driver's license will be accepted as proof of residence. If you are living with someone and they own the house, you will need to complete a Residency Packet.
3. PROOF OF CUSTODY - A parent or legal guardian must register the student. If a child does not reside with both natural parents, we must have proof of custody (divorce papers, legal document - formal court order). NOT ACCEPTED are: Divorce papers awarding "visitation or shared or joint custody where the enrolling parent is not the primary custodian, a petition for custody through Family Court, a power of attorney, temporary transfer of parental rights, or other guardianship papers.
4. PROOF OF GRADE LEVEL - Withdraw from previous school or previous year's report card.
5. SOCIAL SECURITY CARD
6. BIRTH CERTIFICATE
7. Copy of Enrolling Parent's Driver's License (must have the correct address)
8. ANY OTHER RECORDS - (Exceptional Education, Gifted, etc....)

* Donation - $10.00 of your donation will be placed in a non-public fund for school wide activities, reward programs, incentives, etc. These include SAT Celebrations, Cougar Day, Awards Night, etc. Thank you in advance for your support of CCMS.
** STI Home gives parents access to important information about their children directly from their computer, anytime, anywhere. Parents and students can view current class averages, homework assignments, class attendance, discipline, schedule, and school events. This program requires an identification number and PIN that will be provided to those parents that choose to pay for these services.
