Spring Break  March 15th - March 19th.

Character Education Word of the Week is  

Perseverance

Perseverance is commitment, hard work, patience, and endurance.  Perseverance is being able to bear difficulties calmly and without complaint. It is trying again and again.

• If you forgot your money for the tennis team T-shirt fundraiser on Friday, come see Mrs. Cagle as soon as possible today!

• All personal band instruments are to be taken home on Thursday, March 11th in preparation for the move to the new band room.  All instruments are to be brought to school on Monday, March 22nd, the Monday after spring break.  New lockers in the new band room will be assigned during class only to those personal instruments that have an acceptable name tag on the case. Luggage tags may be purchased from Wal-mart, Target, K-mart, or like store at a very reasonable cost.  A piece of tape on the case is not acceptable.  this requirement is in your Procedures and Expectations gien to you at the beginning of the year.  It is not require that school owned instruments

• To nominate a teacher of the month, tell in 50 words or less why you feel your teacher deserves this award.  Please make sure your name, grade, and the school you attend are on your entry.  Then drop it in the box located in the Main Office.  Thanks, the City of Clay School Committee.

NEW STUDENT REGISTRATION

THE FOLLOWING DOCUMENTS ARE REQUIRED BY ALL STUDENTS REGISTERING AT CLAY-CHALKVILLE MIDDLE SCHOOL

• Lease or deed to residence (or mortgage statement or payment book) in the name of the enrolling adult or spouse.

• Two CURRENT primary utility statements (power, gas, water, phone - landline only) of enrolling adult or his or her spouse, identifying the residence as the service address. WE WILL NOT accept cable bills, deposit slips, checks, or driver's licenses as proof of residence.

• Copy of driver's license of parent / guardian.

• Completed registration forms. This includes all work and emergency names and numbers.

        New Students wishing to enroll will need the documents listed above as well as:

• Current Original State of Alabama Immunization Form

• Original or copies of child's birth certificate and social security card.

• Proof of Custody - A parent or legal guardian must register the student. If a child does not reside with both natural parents, we must have proof of custody (divorce papers, legal document - formal court order ). NOT ACCEPTED are: Divorce papers awarding "visitation" or shared or joint custody where the enrolling parent is not the primary custodian, a petition for custody through Family Court, a power of attorney, temporary transfer of parental rights, or other guardianship papers.

• Proof of Grade Level - withdrawal form from previous school or previous year's report card.

• Any other Records (Exceptional Education, Gifted, etc...)
